Scheduling is easy. Follow these simple steps to complete the scheduling process.
Provide a copy of the patient's face sheet or insurance cards (both are better if possible)
Download the paper form or complete our online scheduling form. An additional signature page must be signed when using the online form.
A signed Doctor's order is required for Texas Healthspring patients, or if the facility SLP does not sign the intake form.
